Please include this health form when registering for Day Camp, She & Me, and Troop Core Camp. Girls who are attending Resident Camp (Sunday-Friday) do not need to submit this form.

Camp Golden Valley Resident Camp Girl Health History and Exam Form (38 KB)
This form will be mailed out in the confirmation packet to all girls attending Resident Camp (Sunday-Friday). New Health Forms should be brought to camp; please do not mail to the office. Girls who are attending Day Camp, She & Me, or Troop Core Camp do not need to fill out this form. Please only print this form if needed before the confirmation packets are mailed. If a physical has been completed within the past 24 months and we have it on file, we will have it ready at check-in.
